• Date, moon phase, and moon's sign. Tap tonight's phase under the eight moon icons and pick the sign from a dropdown.

• Spell title and my intention. A name you'll recognize in a year, and what you're asking for in your own words.

• What I used. Four lines, with colors and amounts so you can do it again.
• What I did. Four numbered lines for the steps, in order.
• What I said. Three lines for the words, whether spoken, chanted, or written on the petition.
• Signs and synchronicities. Two lines for what showed up in the days after.
• Follow-up. A date and four choices: it worked, partly, not yet, I let it go.
• Notes. A box for what you'd change next time.

The page has real form fields. On a tablet or computer every line is a text box, every square is a checkbox, and the moon phase and follow-up choices select with a tap. It works in Acrobat, Preview, Apple Books, PDF Expert, Xodo, and most browsers.

If you'd rather write by hand, print it. The fields disappear on paper and you're left with a clean sheet of writing lines and boxes. It prints on plain US Letter paper and works loose in a binder, which is the closest thing to a Book of Shadows you can also reorganize. Stylus apps like GoodNotes and Notability treat it like paper too.

A spell is an experiment, and an experiment without notes is a guess you can't learn from. Coming back a few weeks later and marking what came of it is how you find out which workings suit you. "I let it go" is on the page on purpose. Some workings are released rather than finished, and the record should allow for it.
