• Date and deck. The deck matters, because the same card reads differently across decks.
• The question. What you asked the cards, or what was on your mind when you shuffled.
• The spread. One card, three cards, or my own spread.
• Three card slots. Each has a "this card is for" line, so the positions mean whatever your spread means: past, present, future; now, next week, next month; or three separate questions. Plus a card line and a reversed box.
• My intuitive read. Three lines to write down your take before you research.
• Book meaning. Three lines for what the guidebook says, and where it agrees with you and where it doesn't.
• Revisit. A date line and four choices: spot on, partly, not really, too soon to tell.
• Notes. A box for symbols that stood out, a colour, a figure's expression, anything else.

The page has real form fields. On a tablet or computer every line is a text box, every square is a checkbox, and the revisit choices select with a tap. It works in Acrobat, Preview, Apple Books, PDF Expert, Xodo, and most browsers.

If you'd rather write by hand, print it. The fields disappear on paper and you're left with a clean sheet of writing lines and boxes. It prints on plain US Letter paper and works loose in a binder or trimmed for a journal. Stylus apps like GoodNotes and Notability treat it like paper too.

A reading is a guess about a situation. Coming back a week later and noting how it played out is how a guess becomes learning, and it's the part most tarot journals leave off. This page makes it one line, so it actually gets done.
